The target population that may receive Mental Health Targeted Case Management (MHTCM) as part of the Texas Medicaid Program are clients, regardless of age, with a single diagnosis of chronic mental illness or a combination of chronic mental illnesses as defined in the latest edition of the American Psychiatric Association’s (APA), Diagnostic and Statistical Manual of Mental Health Disorders (DSM) and who have been determined via a uniform assessment process to be in need of MHTCM services. MHTCM services are furnished to assist clients in gaining access to needed medical, social/behavioral, educational, and other services and supports.
Intensive community-based one on one service provided to clients who require services to control acute symptoms that place the client at immediate risk of hospitalization. Crisis Intervention services are available 24 hours a day, seven days a week, to provide for rapid response, assessment and early intervention to individuals experiencing a behavioral health crisis. Services are deployed in real-time to the location of the individual experiencing a behavioral health crisis.
Individualized screenings are clinical assessments that include symptom assessments, medication assessments, and financial eligibility screening to improve access to affordable mental health services for low-income families. Based on the screening, individuals are provided with a service plan. Individual therapy, family, and group therapy are provided by licensed mental health professionals.
Skills training and development is training provided to an eligible client, the LAR, or primary caregiver on behalf of an eligible client. The training addresses: Serious mental illness and symptoms related problems that interfere with the client's functioning and living, working, and learning environment. Skills training and supportive interventions focus on the improvement of communication skills, appropriate interpersonal behaviors, and other skills necessary for independent living or, when age-appropriate, functioning effectively with family, peers, and teachers.
Psychosocial Adult Rehabilitation Program is a non-residential program that provides training and support to enable clients with significant functional limitations to achieve and maintain community stability and independence. Psychosocial rehabilitative services include independent living services, coordination services, and employment, housing, and medication-related services.
Medication management is the process of organizing and tracking a patient’s medications to ensure safe and effective use. This includes prescribing, dispensing, administering, monitoring, and adjusting medications as needed. It also involves regular communication between healthcare providers, patients, and caregivers to ensure proper medication use and prevent adverse effects
Parenting Classes provide parents with a comprehensive understanding of how to address their children's mental health, involvement, and other concerns that can impact family dynamics. These classes are designed to equip parents with essential skills and knowledge to better navigate and support their children's mental health and behavioral well-being.
We advocate tirelessly for the needs and rights of children and families. We take the time to listen to clients' concerns and needs, ensuring we understand their unique situations. We advocate for clients particularly when they face challenges in communicating their needs or rights. We help clients understand and navigate complex mental health systems, including healthcare, education, and social services. We help clients contact relevant people, or contact them on their behalf. We accompany clients and support them in meetings or appointments.
